To reach the Three Fingers you need to get into the sewers under Leyndell, Capital City. Once in town, head to the wing of the giant dragon statue. Instead of climbing it, go to the area under the statue and look for the fountain with the ladder attached to it. Go down the well and follow the path to the Place of Mercy.
Traverse the sewers until you find the Forsaken Depths Site of Grace. Next you must fight Mohg the Omen. Once he’s defeated, you need to attack the altar he was standing in front of. This will move it out of the way, revealing a hidden path. Follow the path down until you reach a thin wooden beam. Instead of going down, you have to fall from the beam to the others below. When you’re done with the beams, find the next coffin sticking out of the wall and start using it to come down.
Once you hit the ground, the ground beneath you will give way and you will fall into a new area below. In front of you is a red and black door with the message that only the wretched can open it. To open the door, remove all of your armor. When you go to the door again, you will be asked to open it. You will then get a cutscene where your character gets grabbed by the three fingers.
